Addiction Recovery Services

At Clarity Healthcare, we know there is no single pathway to recovery. That’s why we offer the individualized care and support you need as you start your journey to a healthier you. We offer the following programs and services to help you take it one day at time.

Addiction Medicine Program

The Addiction Medicine Program (AMP) is the use of medications in combination with counseling and behavioral therapies, to provide a “whole-person” approach to the treatment of substance use disorders. Research shows AMP’s approach can successfully treat these disorders and help sustain recovery.

Eligibility and Screening: A diagnosis of Substance Use Disorder and patient ’s desire for AMP services is needed.

A care coordinator will schedule an appointment for a comprehensive assessment. if eligible for services, a physical examinations with a primary care provider will be completed. An appointment with the AMP prescriber will be scheduled after.

The AMP provider develops a treatment plan in collaboration with the patient and AMP team of professionals.

Patients are required to see the AMP provider as recommended and patient is also expected to follow any other recommendations (e.g. counseling, self-help groups) from the AMP provider.

Additional Levels of Addiction Recovery Services

Provides intensive outpatient services to address substance use problems in an outpatient setting.

  • Adult Intensive Outpatient:
    • Serving adults age 18 and above
    • A minimum of 9 hours per week of group setting
    • Weekly individual sessions with a counselor/therapist

Provides services to address substance use problems in an outpatient setting

  • Adult Outpatient:
    • A maximum of 7 hours per week of group setting
    • Weekly/bi-weekly/monthly individual session with a counselor/therapist
  • Adolescent Outpatient:
    • Serving adolescents age 12-21
    • Weekly/bi-weekly/monthly individual session with a counselor/therapist

Provides services to address substance use problems

  • Bi-Weekly/Monthly individual sessions
  • Serving adolescents and adults ages 12 and above

SERVICES PROVIDED:

  • DUI Alcohol and Drug Evaluation Uniform Report
  • DUI Alcohol and Drug Evaluation Uniform Report Update
  • Investigative Summary
  • Drivers Risk Education (DRE) Class

DUI PROGRAMMING:

  • Minimal Risk – Requires 10 hours of DRE
  • Moderate Risk – Requires 10 hours of DRE and 12 hours of Early Intervention Services
  • Significant Risk – Requires 10 hours of DRE and a minimum of 20 hours of treatment
  • High Risk – Requires 75 hours of treatment

  • A DUI Alcohol and Drug Evaluation Uniform Report - $135
  • DUI Alcohol and Drug Evaluation Uniform Report Update - $165
  • Investigative Summary - $165
  • Drivers Risk Education (DRE) Class - $150 (plus $10 for material)
  • Secretary of State Treatment Verification - $200
